Ts’il Kaz Koh Community Health
The TKK Community Health Program and Patient Travel works alongside FNHA, Carrier Sekani Family Services, and Northern Health to support the health and well-being of community members through accessible health services and resources. The program assists with patient travel, health benefits navigation, resource sharing, wellness education, and other opportunities that help strengthen physical, mental, and emotional wellness within the community.
Patient Travel
The Patient Travel Program manages medical transportation benefits administered through FNHA benefits and funding agreements with recipients. The purpose of the program is to support eligible members with out-of-town travel for medical appointments and healthcare-related services.
To access the program members are required to be registered with FNHA and may be asked to provide the following:
A completed medical form (one-time requirement)
A doctor’s note or referral
Appointment confirmation
Confirmation of appointment attendance following the appointment
The TKK Community Health Representative works with members to access and complete the required documentation in a way that is supportive and accessible. Assistance can be provided by phone, email, or in person, while ensuring all personal and medical information remains confidential.
Community Health
The Community Health Program is responsible for planning and delivering meetings, workshops, and other health opportunities that support the physical, mental, emotional, and spiritual well-being of community members. The program works alongside the wellness team to address health challenges affecting the community and to provide meaningful health and support services where needed.
The program also plays an important role in fostering cross-cultural understanding between community members, clients, patients, and healthcare professionals from different backgrounds and places of origin. Through advocacy, education, and support, the Community Health Program helps bridge gaps in understanding and access to care.
At the heart of the program is the goal of empowering community members to take control of their health by providing access to the tools, resources, knowledge, and supports needed to make informed decisions and build healthier lives for themselves, their families, and future generations.

If you are eligible for the Health Benefits Program, you must first enrol in the program to access benefits and coverage.
If you need to enrol for the first time, or you want to confirm that you are already enrolled, have your status number ready and call Health Benefits at 1-855-550-5454.

Program Goals from Comprehensive Community Plan
Promote health and wellness activities through the development of different programs and groups (e.g., yoga, hiking, walking, snowshoeing, etc.) to bring people together, encourage physical activity, and foster member connection.
Expand health coordinator role to include informing members regarding access, education and participation in programs and services (e.g., how to access to doctors and other healthcare providers such as physiotherapists, chiropractors, massage therapists, and the Aboriginal liaison).
Improve Elder care by training more caregivers, hiring more staff for personalized attention, and focusing on preventive health measures such as regular health screenings and tailored exercise programs to keep Elders healthy and promote quality of life.
Complete a feasibility study to understand the potential of hiring a home support worker to support members who need assistance with daily living.
Identify a connection to a health care advocate who can answer health and medical questions, help members complete forms, direct members to health services, and advocate for members’ medical needs when necessary.
Host one (1) health fair for members each fiscal year (e.g., promotion of health services available to members).
Make sure all members have equal access to funding opportunities to use and benefit from health programs and services.
